A 19–year-old lady reported to the clinic with the chief complaint of discoloration of teeth. The overall oral health of the patient was favorable. On clinical examination, moderate yellowish bands of discoloration were observed. Following this, the patient was reviewed with the options and details involving bleaching. After having a detailed discussion the patient felt that in-office bleaching would meet her needs.

Radiographic examination was carried out for checking the vitality of the teeth and ruling out the presence of any periapical pathologies.
No periapical radiolucency was observed on the radiograph. Additionally, the patient gave no history of tooth sensitivity.

For better accuracy, pre-operative intraoral images were recorded. These images can also be used as a resource to control the shade progression.

Oral prophylaxis is a thorough scaling and polishing that was carried out for all the teeth before starting the bleaching procedure.

During the start of the bleaching procedure, the patient is asked to wear protective eyewear. A self-supporting cheek retractor was placed in the patient’s mouth covering the upper and the lower lips.
 

After removing the Luer lock cap from the gingival barrier syringe the tip was attached securely to it. The teeth and the gums were rinsed and air-dried thoroughly, isolation was maintained appropriately. The gingival barrier was dispensed along the gingival margin at a continuous speed overlapping approximately 0.5mm onto the enamel.

The resin was light-cured for 20secs in a scanning motion.
 

A 0.5 - 1mm thick layer of the gel was obtained after mixing the powder and the liquid was applied to the labial/front surface of the teeth. With continuous curing, the gel remained on the teeth for 20-25 minutes and saliva was suctioned periodically as needed.

The gel was then removed completely from the surface of the teeth following which the teeth were thoroughly rinsed. The gingival barrier was removed entirely. The patient is asked to rinse thoroughly. The following is the immediate post-operative image showing whiter teeth and significant change in the shade.
 

The patient was recalled after a period of 7 days to evaluate the results. Using a standard visual examination, the shade change was evaluated. A noticeable shade change had occurred. The patient noticed a marked improvement with comfort and was very ecstatic with the final outcome.
